Medical Records
Medical records comprehensively document the patient’s clinical history throughout their entire course of care in the hospital, including admission details, diagnostic findings, treatments administered, procedures performed, and progress notes. These records serve as official and legal documentation of the patient’s confinement, diagnoses, and medical management. They are essential for continuity of care, clinical evaluation, and may be used for administrative, legal, and insurance purposes when required.

Clinical Abstract
Released 4 working days upon request -
Certificate of Confinement
Available upon request during confinement period only -
Medical Certificate
To be released during (ONCE PATIENT TAGGED AS MGH AND WITH FINAL DIAGNOSIS) and after discharge -
Laboratory Result
To be released before patient discharge -
Video Procedure Copy
1 day upon request -
Discharge Summary
4 working days upon request after discharge date -
Birth Certificate
Bring the fill out Provisional Certificate to Medical Records
- If married - kindly attach copy of Marriage Certificate / Marriage Contract
- If not married - kindly bring 1 valid ID of mother

Death Certificate
Bring the Death Certificate Releases Form from the Cashier. Make sure to know the information of the patient (e.g. Full name of the Parents, Birthday, Name and Address of Cemetery)

- Certificate of Confinement
Available upon request during the confinement period.
- Clinical Abstract
4 working days upon request.
- Discharge Summary
4 working days upon request after the discharge date.
- Laboratory Results
Released after patient’s discharge, and upon formal request.
- Medical Certificate
Can be released once tagged as MGH* and with final diagnosis and can also be released after discharge.
- Video Procedure Copy
1 day upon request.
- Birth Certificate
Requirements: bring the filled-out provisional birth certificate to medical records.
- If married – kindly attach marriage certificate / marriage contract
- If not married – kindly bring 1 valid id of the mother
- Death Certificate
Requirements:
- Bring the death certificate release form from the cashier
- Provide patient information (e.g., Full name of the parents, birthday, name and address of cemetery).
- If first degree relative is absent: Next of kin may be authorized to sign and claim. Both representatives must submit a photocopy of a valid id, birth/marriage certificate, or legal proof of relationship.
PROCEDURES IN REQUESTING OF MEDICAL RECORDS
During Admission
- Please visit the Nurse Station and inform the staff of your required medical records.
- The Ward Assistant will secure the requested medical records by entering your request into the Medical Records Tracking System Logbook – STN. Please ensure you sign the request form.
- Turnaround times may vary depending on the request document. If the patient is still admitted, you may follow up directly at the nurse station.
After Discharge from the Hospital / Emergency - Outpatient
- Kindly visit the Medical Records department and inform the staff of the specific medical records you need.
- The Medical Records staff will then obtain the requested medical records for you by entering your request in the Medical Records Tracking System Logbook. Please ensure you sign the request form.
- You will receive the records based on the standard processing time. The Medical Records will inform you via text once the document becomes available.
Note:
- The patient must present a valid ID upon releasing the Medical Information
- If the patient is unavailable, the authorized representative must present a valid ID, an authorization letter and valid ID of the patient
- This protocol ensures strict confidentiality and guarantees that documents are released only to a legally qualified representative.
